National Healthcare Decisions Day is an initiative encouraging patients to express their healthcare wishes through conversations and the completion of advance directives and hospital permissions. On this day throughout the country, healthcare providers, professionals, chaplains, attorneys, and others will participate in a massive effort to highlight the importance of advance healthcare decision-making.
What is a living will?
Living wills/advance directives are actually a set of documents: a
living will/advance directive and a medical durable power of attorney.
These documents help direct your medical care if you become
incapacitated. Our living wills and advance directives services provide
free documents specific to your state. We also provide a member benefit
of one-on-one counseling on creating and implementing your living
will/advance directive.

What are hospital permissions?
Unmarried partners may need prior hospital visitation authorization from their partner. This document let's the hospital and healthcare workers know that should you become ill or incapacitated it is your wish that certain named persons be given first preference to visit you.
