Choosing a doctor, medical team, facility and hospice that are supportive of your end-of-life choices is important. If medical aid in dying is an important end-of-life care option for you, this online tool is designed to help you choose a facility, medical team and/or hospice that will support your end-of-life choices.

Please see our Introduction to Using the Find Care Tool to see four (4) important steps in identifying a facility, medical team and/or hospice that will support your end-of-life choices. This document will help guide you through this very important process.

Currently this tool identifies hospitals, medical clinics, and hospices that have adopted policies that support patient end-of-life choice in the states of California, Colorado, Hawai‘i, Montana, New Jersey, Oregon and the District of Columbia.

Please check back frequently as we are constantly adding new facilities to the Find Care tool.

*If you work for a health system, hospital, medical clinic, hospice or retirement community that is supportive of patient choice and you would like to see your facility added to this tool, or if you are a patient who would like to share your experience, please let us know by emailing us.
