The Provider Orders for Life Sustaining Treatment (POLST) is a portable medical order that can give patients with serious illness increased control over the treatment they do and do not want to receive at the end of life. The POLST helps to ensure the patient’s wishes are conveyed to emergency services and other medical providers.
Compassion & Choices Medical Director, Dr. Susan Wilhoit, a palliative care physician, will explain how the Minnesota POLST fits into end-of-life planning. She’ll discuss who needs a POLST and how a POLST differs from an Advance Directive.

We’ll walk you through this new tool to share how we are working to transform how people live and die with dementia to ensure they are empowered and supported in getting the care they want, or avoid the care they don’t want.
